Hospital Discharge to Residence Care of Self

Dispatched to #### in response a Discharge from hospital to back to origin.

On arrival the scene was surveyed and found to be safe, Body Substance isolation  taken by all members of the crew involved

Ambulance was exited and Entry was made into the Hospital, Contact was made with the Nurse in charge, the nurse was able to Provide a report on the patient as well as paperwork on the patient for discharge.

After transfering the patient , the patient was secured with 2x stretchers side rails and all 4 seatbelts.

the patient was then adjusted to a Positions of Comfort patient was covered with blankets other comfort measures taken at patients descension.

Assessments was preformed and Vitals was taken. 

Patient was then taken to ambulance and safely loaded without issues.

Transport started.

 patient was unloaded from the ambulance and entry was made and patient taken inside via the Stretcher. 

Patient was taken to assigned area and unsecured from stretcher

Patient was transferred without issues.

Patient was left in care of Self under own recognizances , it was assured that the patient was left with access to a phone, fluids , and any requested necessity's before departure , Patient was able to sign for self.

NO issues on transport. The patient was left without incident.

after the patient was dropped of the Unit was cleaned and Report finished , then the unit went back into service.
